Qualifications

• Must be a licensed with the State of Florida as an LPN, and must maintain and a current and unencumbered license

• Must have a valid driver’s license

• Must have current CPR/First Aid certification and maintain certification

• Must maintain professional CEU’s for license renewal

• Possess sensitivity and aptitude for working with the elderly of all conditions

• Possess skills to communicate effectively with residents, family members and coworkers.

• Must be able to prioritize and organize work effectively and efficiently

• Able to work irregular hours to include, nights, weekends, holidays and some early morning shifts

Responsibilities

• The LPN will assist with planning, coordinating, and managing the care services and act within the regulations of the facility.

• The LPN is responsible for the overall direction, coordination and monitoring of care service delivery alongside the monthly RN review

• Sanitizing, disinfecting, or cleaning facility to include resident rooms, kitchen and commonareas. 

• Assisting with resident ADL’s to include but not limited to bathing, dressing and assistingwith activities

Job Description 

Primary Purpose:

This position will ensure the health and safety of all residents are met while maintaining patients comfort and well being. The LPN will assist the Administrator and other care team members with coordinating resident care, ensuring ADL’s are met, providing physical assessments and any other duties as permitted within their scope of care. LPNs will assist and monitor resident medications and be able to document MOR forms. LPNs will ensure that they are following all rules, guidelines and regulations that govern residential care facilities.  

LPNs will participate in surveys and inspections made by authorized government agencies.  LPNs will be responsible for updating resident admission plans, care plans and discharge plans. 

LPNs will work with additional providers and receive telephone orders that will be updated in the residents’ care plan as well as communicated with other parties needing to know.   LPNs will be responsible for completing accident/incident reports as needed. LPNs will be responsible foridentifying and participating in process improvement initiatives that improve resident care and well being, workflow and client experience.  LPNs will assist with daily housekeeping activities of the facility as well as daily meal prep and service. LPNs may need to help with transport of patients to appointments and activities as well as run center activities.   LPN’s may be responsible for overseeing other caretakers such as MA’s and CNA’s as needed. This job description is not all inclusive.  LPNs may be required to complete other duties as assigned.
